How to Apply for Food Stamps in Kentucky

      Download and print a food stamps application from the Cabinet for Health and Family Services (see Resources). The application is available in various languages as a Word document.

    • 2). Complete the food stamps application. Do not skip any sections. If you need assistance in completing the application, contact your local Department for Community Based Services (DCBS) office. A representative can answer any questions concerning the application.

    • 3). Gather personal documents needed to support the information you entered on the application. You need to provide your driver's license or picture identification, Social Security card and proof of residency. Proof of residency can include a lease or utility bill.

    • 4). Gather proof of income. Your eligibility to receive assistance is partially based on your monthly income. In order to make a determination on your case, a caseworker must be able to verify the income information you provide on your application. You can provide your last four paycheck stubs, the previous year's tax return or W-2 forms to show how much income your family receives.

    • 5). Submit your application to your local DCBS office. Take your personal documents and proof of income with you. If a caseworker is available, you may be able to interview to determine if you are eligible for food stamps. A DCBS representative may make copies of your documents when you submit the application.

    • 6). Attend your eligibility appointment. A letter will be mailed to you with an appointment time. If they didn't make copies of your personal documents and proof of income when you submitted your application, take the documents to the interview. Missing your appointment could result in a denial or delay in processing your application.

    • 7). Wait for a determination letter. The letter will be mailed to the address provided on your application. If you were approved for food stamps, an Electronic Benefits Transfer (EBT) card will be mailed to you. You can use the card to purchase food at authorized dealers.
