How to Report Fraud to the DCAA
- 1). Visit the US Department of Defense Office of Inspector General website. Type "Suspected Irregularity Referral Form" into the search text box on the home page. Click the "Search" tab.
- 2). Select the Suspected Irregularity Referral Form link on the next page. Download and print the form.
- 3). Complete the form in its entirety. Note your personal contact information and the information related to the contractor. Include the government agencies harmed by the fraud with specificity and detail.
- 4). Type out a separate sheet listing all of the suspected violations and irregularities. Identify who is alleged to be involved, the details of the situation, how the fraud is being committed and how much money is at stake. Identify evidence and witness testimony that can support your claim. Sign and date all documents and have a branch manager for the DCAA sign as well.
- 5). Submit two identical copies of forms: one to the DOD and one to the branch manager's office at the DCAA.
