How to Apply for the Home Energy Assistance Program in Ohio
- 1). Download an application for Ohio HEAP from the Ohio Department of Development website.
- 2). Complete the application. Do not leave out any information or your application will not be processed.
- 3). Photocopy documents that show the income for all members of your household, such as paycheck stubs, award letters from the Social Security Administration, tax documents or proof of Worker's Compensation or unemployment benefits. If you have no income at all, write a statement explaining how you maintain your household and pay your bills and sign it.
- 4). Photocopy documents verifying citizenship status for all members of your household, such as birth certificates or passports.
- 5). Photocopy documents indicating someone in your household has a disability, if someone in your household is indeed disabled. In that case, your household may receive additional money. Examples of proof of disability include an award letter for Social Security Disability Insurance, a benefit letters from the Veterans Administration or a written statement from a physician.
- 6). Photocopy your most recent heating or fuel bill and your most recent electric bill.
- 7). Mail the completed application and all documentation to:
