How to Correct Birth Certificates From Kentucky
- 1). Carefully examine your birth certificate and determine any changes that need to be made.

Frankfort, KY 40621 - 3). Explain to the registrar's office all of the reasons why you need an amendment to your birth certificate.
- 4). Ask the office to send you all of the appropriate forms you will need. For example, if a father has not been designated on the birth certificate, a form VS-8 will need to be filed. Unmarried women must fill out a form VS-8B to declare paternity, and both natural parents must agree to it. Form VS-8C is used to correct the father on the certificate in the event that the wrong person was listed as father.
- 5). Request that obvious errors such as misspellings be corrected. Obvious errors are supposed to be corrected within one year of birth. The office may be willing to correct the error or you might have to petition your local court office to have the correction made.
- 6). Obtain a court order for name changes, date of birth changes and to change any amendment that has already been made to a birth certificate.
- 7). Complete an affidavit for all other changes. The registrar's office can provide a template for an acceptable affidavit.
- 8). Mail all required documents and all supporting evidence you have to the Kentucky State Registrar Office, along with a money order for $10. The money is required to receive a corrected copy of your Kentucky birth certificate.
