Medication Errors Happen because of Complexities!

101 37
In the busy, daunting, and complex world known as today’s United States Healthcare system, the simple fact is healthcare providers do not have the time in a patient visit to physically review all of a patient’s medications or call the pharmacy each time to see which prescriptions have been picked up. In addition, in the case of the elderly, keeping track of a patient’s medication profile becomes increasingly difficult due to the fact they may be on several different medications at one time. Furthermore, with less time, more medications prescribed, and more administrative work with healthcare insurance companies, it is no wonder the Food Drug and Administration (FDA) reports “adverse events from drug use result in more than 4 million visits to emergency departments, doctors’ offices or other outpatient settings annually and 117,000 hospitalizations each year.”

In today’s world of Health Maintenance Organizations (HMOs), Preferred Provider Organizations (PPOs), Private-Fee-For-Service (PFFS), Point of Service (POS), Original Medicare, Medicare Advantage, State Medicaid plans, or Private Medicaid plans and the list only continue! The simple fact is it is impossible to think a healthcare provider can keep track of a patient’s profile when they are changing from insurance to insurance, doctor to doctor, and being prescribed from one medication to medication.

The sad and unfortunate truth is that each of these events compound and increase the incidence of medication errors within this sector. The good news, however, is that those within the sector are aware of medication errors taking place. Organizations such as the Institute of Medicine (IOM) are monitoring and educating of the dangers of medication errors. The FDA is launching new programs such as the Safe Use Initiative program to collaborate and educate healthcare providers and the community about the dangers of medication errors. Finally, health reform is calling for greater expansion of electronic health records, and offering incentives to healthcare providers who convert to electronic medical records.

The problem arises amongst those healthcare providers who are unable to spend the large amounts of money required to implement an electronic health record system. Those healthcare providers who are looking for change as well as additional benefits for their patients are looking towards Ncapsul, Inc, which specializes in “…Making Medicine Simple.” Ncapsul provides a Capsulcard (durable wallet medication record) to list the medications they currently are taking along with their providers and any allergies.
