How to Obtain a Credit Freeze in Arizona
- 1). Develop a letter to the credit bureaus stating that you want to place a credit freeze on your credit file. Include your name, address, social security number and date of birth in the letter. You will also need to provide a copy of your driver's license and proof of residence, such as a mortgage note or rental agreement. You will also need to provide a copy of a government-issued identification car,d such as a driver's license, military ID or social security card.
- 2). Mail the letter to the credit bureaus with a $5 fee for each bureau unless you have a investigative or police report showing potential identity theft. Pay the fee if required by check, credit card or money order. The three credit bureaus are Experian, Equifax, and TransUnion. You can choose to freeze only one credit bureau or all three. The credit bureaus must mail you confirmation of the credit freeze to the address you provided within 10 days of placing the credit freeze on your credit file. There will be a PIN number in the confirmation letter which you must have to remove or temporarily lift the credit freeze in the future. Contact the credit bureaus if you do not receive a confirmation within 17 days. It may take seven days to place the credit freeze and 10 days for confirmation to be mailed to you.
