How to Make a Name Change
- 1). Print out the Application for a Social Security Card (see Resources), and fill it out. This form is not only used to obtain a Social Security card; it is also used to make name changes.
- 2). Obtain proof of citizenship, proof of identity and proof of the name change (marriage certificate, birth certificate or other legal document). The Social Security Administration will only accept original documents, not copies.
- 3). Take your completed application and original documents to your county's local Social Security office. You can mail the information if it is not possible for you to get to the office in person.
- 4). Wait two weeks to receive your new card with the name change in the mail. If you mailed in your application, you will receive your original documents in the mail separately.
