How to Order a Replacement DD214
- 1). Enter the words: U.S. National Archives and Records Administration (NARA) in your computer's search bar. The NARA home page has a horizontal red bar titled "Request Military Records" located at the bottom of the page. Click the red bar.
- 2). Key in data and customize the form to fit your needs. Complete the four steps needed to navigate through the system. Prompts, by the system, will assist you.
- 3). Refine your search. Select form DD214 on the drop down menu in the second step of the online request form.
- 4). Print a copy of your form. Sign and date the form where indicated. The veteran or next of kin must sign each request for a replacement form DD214. A veteran's next of kin must provide proof of death of the veteran.
- 5). Mail or fax the signed and dated signature verification form to the U.S. National Archives and Records Administration, 8601 Adelphi Road, College Park, MD 20740-6001.You will have 20 days from the date of your computerized request within which to mail the form.
